Appointment Timeframes:
Acute Appointment (ACUTE): Same Day
Procedure (PROC): 28 days or provider designated duration
Future (FTR): 7 days or provider designated duration
How To Do a Prescription Renewal:
Call your Medical Home Port Team or sent them a message through
MHS Patient Portal and provide the name of all medications and dosages, along with your full name, sponsor's last 4 SSN and a good contact number. Once your medication has been renewed by your Medical Home port team provider, a nurse will call and let you know when you can go to pharmacy to have your prescription filled.
How To Communicate with your Provider:
You can access the
MHS Patient Portal link to communicate with your Medical Home Port team, or view your most recent lab results.
