How do I make an appointment?
By calling our Appointment Call Center at 624-2273 or 095-56-2273. You may also use the Appointment Call Center to make, cancel, and reschedule appointments or leave a message for your medical provider.
Check-In Time
You need to arrive at least twenty minutes prior to your stated appointment time. If you are more than 10 minutes late, you will see the provider at an earliest convenience, or may be asked to reschedule. Parking varies at each facility, and may be limited, so please plan accordingly.
Dress Code
Active Duty personnel must be in the prescribed Uniform of the Day to be seen at our facility, unless on leave or in a liberty status.
